Individual & Family
Are you ready for personalized care at an affordable price point? We would love to have you join the Ascent family! Memberships include up to 10 office visits, including wellness, follow-up visits, acute care, or sick visits per year. Subsequent visits are just $20 at the time of service.
Includes yearly wellness visit with necessary labs (including CBC, CMP, TSH, for adults, PSA for men and PAP for women when indicated) and an EKG (if indicated).
Individuals - $79 per person, per month
Children - $29 per child, with at least one parent as a member
*One-year membership commitment
